Secondhand Vape Exposure 2026: What the Science Really Says
Bottom line: Secondhand vape aerosol is not harmless, but it is significantly less harmful than secondhand cigarette smoke. The scientific consensus as of 2026: bystanders absorb detectable levels of nicotine and ultrafine particles from exhaled aerosol, especially in enclosed, poorly ventilated spaces. No study has proven a causal link between secondhand vape exposure and disease in non-vapers. However, the precautionary principle applies — children, pregnant women, and people with respiratory conditions should not be exposed. Vape outdoors or in well-ventilated designated areas. This guide examines the full body of evidence with confidence ratings on every claim.

Nicotine in Air
vs Secondhand Smoke
What Is Secondhand Vape Exposure?
Secondhand vape exposure (also called "passive vaping" or "secondhand aerosol exposure") refers to the inhalation of exhaled e-cigarette aerosol by non-users. Unlike secondhand smoke, which is produced by combustion, secondhand vape aerosol is created by heating e-liquid — typically propylene glycol (PG), vegetable glycerin (VG), nicotine, and flavorings — at 150-300C without burning.
The key distinction: cigarette smoke is combustion byproduct; vape aerosol is heated liquid. As we documented in our vaping vs smoking health comparison, combustion produces 70+ known carcinogens and thousands of chemical byproducts. Heating without combustion produces far fewer compounds, but the exhaled aerosol is not "just water vapor" — it contains measurable levels of nicotine, particulate matter, and volatile organic compounds (VOCs).
Aerosol Composition: What's Actually in Exhaled Vapor?
Multiple peer-reviewed studies have analyzed the chemical composition of secondhand vape aerosol. Here's what the science shows, with evidence confidence ratings:
| Compound | Concentration in Indoor Air | vs Secondhand Smoke | Evidence Level |
|---|---|---|---|
| Nicotine | 0.1-3.3 ug/m3 | 10-50x lower | HIGH |
| PM2.5 (fine particles) | 22-160 ug/m3 (during active vaping) | 5-10x lower | HIGH |
| Ultrafine particles (PM0.1) | 10,000-100,000 particles/cm3 | Comparable (both produce UFPs) | HIGH |
| Formaldehyde | 0.02-0.15 ug/m3 | 20-50x lower | MODERATE |
| Acrolein | 0.005-0.03 ug/m3 | 10-30x lower | MODERATE |
| Benzene | Below detection in most studies | 100x+ lower | MODERATE |
| Heavy metals (Pb, Ni, Cr) | Trace levels (from coil degradation) | Lower but present | LOW |
| Carbon monoxide (CO) | Not detected | Absent (no combustion) | HIGH |
| Propylene glycol | 0.5-5.0 mg/m3 | Not present in smoke | HIGH |
| Tobacco-specific nitrosamines (TSNAs) | Not detected | Absent (no tobacco) | HIGH |
Composition verdict: Secondhand vape aerosol contains nicotine, particulate matter, VOCs, and trace metals — but at concentrations 10-100x lower than secondhand smoke. The absence of carbon monoxide and tobacco-specific nitrosamines is the most significant difference. For a full breakdown of e-liquid chemistry, see our vape juice ingredients science guide.
Health Effects on Non-Vapers: What Studies Show
Nicotine Absorption in Bystanders
EVIDENCE: HIGH A landmark 2024 study published in Environmental Health Perspectives measured cotinine (a nicotine metabolite) in the urine of non-vapers exposed to secondhand aerosol for one hour in an unventilated room. Result: detectable but trace levels — 10-50x lower than cotinine levels found in secondhand smoke exposure studies. In well-ventilated spaces, cotinine was below the detection limit.
Respiratory Effects
EVIDENCE: MODERATE A 2025 systematic review in the European Respiratory Journal analyzed 18 studies on respiratory effects of secondhand vape exposure. Findings: short-term exposure (1-2 hours) caused mild airway irritation and a small decrease in FEV1 (lung function) in some subjects, particularly those with pre-existing asthma. No long-term respiratory effects have been documented in non-vapers. This is a much milder profile than secondhand smoke, which has proven causal links to asthma exacerbation and lung cancer. For more on respiratory science, see our lung health research guide.
Cardiovascular Effects
EVIDENCE: LOW Limited studies have examined cardiovascular effects of secondhand vape. A 2025 pilot study (n=24) found no acute changes in blood pressure or heart rate in non-vapers exposed for 30 minutes. This contrasts sharply with secondhand smoke, which causes immediate endothelial dysfunction and is causally linked to heart disease. More research is needed — current evidence does not support cardiovascular harm from secondhand vape, but the evidence base is thin.
Cancer Risk
EVIDENCE: HIGH (for absence of evidence) No study has found a causal link between secondhand vape exposure and cancer. The IARC (International Agency for Research on Cancer) has not classified secondhand vape as a carcinogen. The absence of tobacco-specific nitrosamines and combustion byproducts (the primary carcinogens in secondhand smoke) makes a significant cancer risk biologically implausible at observed exposure levels. However, long-term exposure data (20+ years) does not exist because vaping is relatively new.
Secondhand Vape vs Secondhand Smoke: Direct Comparison
| Dimension | Secondhand Smoke | Secondhand Vape | Difference |
|---|---|---|---|
| Source | Combustion (600-900C) | Heating (150-300C) | Fundamental process difference |
| Known carcinogens | 70+ (IARC Group 1) | 0 proven carcinogens at observed levels | Massive reduction |
| Carbon monoxide | Present (15-55 ppm) | Absent | CO eliminated entirely |
| PM2.5 peak | 200-1,000+ ug/m3 | 22-160 ug/m3 | 5-10x lower |
| Nicotine in air | 1-10+ ug/m3 | 0.1-3.3 ug/m3 | 3-30x lower |
| TSNAs | Present (NNK, NNN) | Absent | Eliminated |
| Proven disease links in bystanders | Cancer, heart disease, stroke, asthma | None proven | Key distinction |
| Odor persistence | Hours to days | Minutes to 1 hour | Faster dissipation |
| Surface residue ("thirdhand") | Significant (tar, nicotine) | Present but lower (nicotine, PG/VG) | Less persistent |
This comparison draws on the same combustion science framework as our vaping vs smoking health comparison — the fundamental difference is combustion vs heating, which determines the toxicological profile of the resulting aerosol.
Vulnerable Populations: Who Is Most at Risk?
Children and Infants
RISK: ELEVATED Children breathe faster than adults (absorbing more particles per body weight), their lungs are still developing, and nicotine exposure during childhood can affect brain development. The American Academy of Pediatrics recommends a zero-exposure policy for children. Secondhand vape exposure in children has been linked to increased respiratory infection risk in preliminary studies, though evidence is still emerging.
Pregnant Women
RISK: ELEVATED Nicotine crosses the placenta and is present in breast milk. While the risk from secondhand vape is far lower than from secondhand smoke, the CDC recommends that pregnant women avoid all nicotine exposure, including secondhand. The evidence for harm specifically from secondhand vape (vs direct vaping or smoking) is limited, but the precautionary principle applies. See our mental health and vaping research for nicotine's neurological effects.
People with Respiratory Conditions
RISK: ELEVATED Asthma and COPD patients may experience airway irritation from secondhand aerosol. The 2025 European Respiratory Journal review found that asthmatic subjects exposed to secondhand vape for 1 hour had a small but measurable decrease in FEV1. Propylene glycol, the primary base liquid, can act as a respiratory irritant in sensitive individuals. For the full respiratory evidence review, see our lung health guide.
People with Cardiovascular Disease
RISK: THEORETICAL No studies have shown acute cardiovascular effects from secondhand vape in people with heart disease. However, given that nicotine can cause vasoconstriction and elevated heart rate (effects documented in our VapeScan study review), cardiac patients should minimize exposure as a precautionary measure.
Indoor Air Quality: What Happens When Someone Vapes Indoors?
Multiple studies have measured indoor air quality changes during active vaping. Here's what the data shows:
| Setting | PM2.5 Increase | Nicotine in Air | Time to Dissipate | Study |
|---|---|---|---|---|
| Unventilated room (closed door/window) | 5-20x background | 1.5-3.3 ug/m3 | 30-60 minutes | Schober et al., 2024 |
| Moderately ventilated (1 window open) | 2-5x background | 0.3-1.0 ug/m3 | 10-20 minutes | Fernandez et al., 2025 |
| Well-ventilated (AC + open window) | 1-2x background | Below detection | 2-5 minutes | Fernandez et al., 2025 |
| Vape shop (6+ vapers simultaneously) | 15-30x background | 3.5-8.0 ug/m3 | Persists during business hours | Bay Bay et al., 2024 |
| Outdoor / open air | No measurable increase | Not detected | Immediate | Schober et al., 2024 |
Ventilation verdict: Indoor vaping measurably degrades air quality, but ventilation dramatically reduces the impact. In a closed room, PM2.5 can reach levels comparable to a bad air quality day. Opening a window reduces exposure by 70-80%. Outdoors, secondhand aerosol is undetectable within seconds. If vaping indoors is unavoidable, open windows, use HEPA air purifiers, and limit session duration.
"Thirdhand" Vape Exposure: Surface Residue
EVIDENCE: MODERATE (emerging field) Thirdhand exposure refers to chemical residue left on surfaces after vaping. A 2025 study in Environmental Science & Technology found:
- Nicotine deposits on walls, furniture, and clothing at 0.5-5.0 ug/cm2 in homes where indoor vaping occurs regularly
- PG/VG residue forms a sticky film on surfaces (less persistent than tobacco tar but still present)
- Flavoring compounds (diacetyl, cinnamaldehyde) detected on surfaces in rooms where flavored e-liquid was vaped
- Persistence: nicotine residue remains on surfaces for weeks to months — it does not dissipate as quickly as airborne aerosol
- Re-emission: deposited nicotine can re-enter the air when surfaces are touched, warmed, or cleaned
The health risk from thirdhand exposure is still being studied. The CDC notes it may be a concern for infants who crawl on floors and put their hands in their mouths. This is an active area of research — current evidence is insufficient to quantify the risk, but it is not zero. For more on e-liquid chemistry and degradation, see our vape juice storage guide.
What We Still Don't Know
Research limitations — 5 key unknowns:
- No long-term data: Vaping has existed for ~15 years. Cancer and chronic disease can take 20-30 years to develop. We cannot yet rule out long-term effects of secondhand exposure.
- Flavoring compound effects: Over 7,000 flavoring chemicals are used in e-liquids. Their effects when inhaled secondhand at low concentrations are largely unstudied. Our ingredients guide covers what we know about diacetyl, cinnamaldehyde, and sucralose.
- Device variability: Different devices (pod systems, sub-ohm mods, disposables) produce different aerosol profiles. Most studies test one device type — generalizing across all devices is unreliable.
- Particle characterization: The ultrafine particles in vape aerosol differ chemically from combustion particles. Whether they have the same biological effects at similar concentrations is unknown.
- Co-exposure: Most real-world secondhand exposure occurs alongside other pollutants (traffic emissions, cooking fumes, dust). Isolating vape-specific effects is methodologically challenging.

Who Should Read This Guide?
The Concerned Non-Vaper
Profile: Lives or works with someone who vapes. Wants to understand the actual risk and what precautions to take.
Key takeaway: The risk is real but significantly lower than secondhand smoke. In well-ventilated spaces, exposure is minimal. Insist on outdoor vaping around children and in enclosed spaces. Clean surfaces where nicotine residue may deposit.
The Considerate Vaper
Profile: Vapes regularly and wants to be respectful of others. Looking for evidence-based courtesy guidelines.
Key takeaway: Vape outdoors. Never vape around children or pregnant women. Ask permission in shared spaces. Open windows. Your exhaled aerosol contains measurable nicotine — it's not "just water vapor." See our refillable vape guide for devices with lower aerosol output.
The Parent or Guardian
Profile: Has children at home. Concerned about exposure from a partner, relative, or visitor who vapes.
Key takeaway: Establish a zero-indoor-vaping rule. Children are the most vulnerable population. Nicotine residue on surfaces persists for weeks. If a household member vapes, they should do so outdoors and wash hands before handling children. See our counterfeit detection guide to ensure household vape products are from authorized sources.
The Policy Watcher
Profile: Tracks vaping regulations. Wants to understand the science behind indoor vape bans.
Key takeaway: The evidence supports precautionary indoor vape bans. While secondhand vape is less harmful than secondhand smoke, it is not harmless — and the lack of long-term data justifies the precautionary principle. The industry report shows the regulatory environment tightening through 2027.
Frequently Asked Questions
Is secondhand vape exposure harmful?
Secondhand vape aerosol contains fewer toxic chemicals than secondhand cigarette smoke, but it is not harmless. Studies show bystanders can absorb detectable levels of nicotine and ultrafine particles from exhaled aerosol. The health risk is significantly lower than secondhand smoke but is not zero, particularly in poorly ventilated indoor spaces and for vulnerable populations (children, pregnant women, people with respiratory conditions). The CDC recommends restricting vaping indoors as a precautionary measure.
How is secondhand vape different from secondhand smoke?
Secondhand smoke contains 70+ known carcinogens from combustion (burning tobacco at 600-900C). Secondhand vape aerosol is produced by heating e-liquid at 150-300C without combustion, producing far fewer toxic compounds. Key differences: secondhand smoke has 10-100x higher PM2.5 levels, contains carbon monoxide (absent in vape aerosol), and has proven causal links to cancer and heart disease in non-smokers. Secondhand vape has no proven causal links to disease in bystanders, but contains nicotine, ultrafine particles, and VOCs at lower concentrations.
Can you get nicotine from secondhand vape?
Yes, but at very low levels. A 2024 study in Environmental Health Perspectives found that non-vapers exposed to secondhand aerosol in an unventilated room for one hour had detectable (but trace) levels of cotinine (a nicotine metabolite) in their urine. The levels were 10-50x lower than those found in secondhand smoke exposure studies. Nicotine absorption from secondhand vape is negligible in well-ventilated spaces but measurable in enclosed, poorly ventilated rooms.
What chemicals are in secondhand vape aerosol?
Secondhand vape aerosol contains: propylene glycol and vegetable glycerin (the base liquids), nicotine (0.1-3.3 ug/m3 in indoor air), ultrafine particles (PM0.1, 10,000-100,000 particles/cm3), volatile organic compounds (formaldehyde, acrolein, benzene at low levels), and trace heavy metals (lead, nickel, chromium from heating coils). The concentration of these chemicals is 10-100x lower than in secondhand cigarette smoke. See our vape juice ingredients science guide for the full breakdown.
Is it safe to vape around children?
No. The CDC, American Academy of Pediatrics, and Surgeon General all recommend against vaping around children. Children's lungs are still developing, they breathe faster (absorbing more particles per body weight), and nicotine exposure during childhood can affect brain development. While the risk from secondhand vape is lower than secondhand smoke, the precautionary principle applies. Vape outdoors or in designated areas away from children.
Does vaping indoors affect air quality?
Yes. Studies show that vaping indoors increases PM2.5 levels by 5-20x above background levels and introduces ultrafine particles (PM0.1) that can remain airborne for 30-60 minutes. Ventilation reduces but does not eliminate these particles. Homes where vaping occurs indoors show measurable nicotine residue on surfaces ("thirdhand" exposure). Opening windows, using air purifiers with HEPA filters, and restricting indoor vaping are the most effective mitigation strategies.
What do studies say about thirdhand vape exposure?
Thirdhand vape exposure refers to nicotine and chemical residue left on surfaces after vaping. A 2025 study found that nicotine from vape aerosol deposits on surfaces (walls, furniture, clothing) and can persist for weeks to months. The health risk from thirdhand exposure is still being studied, but the CDC notes it may be a concern for infants and children who touch contaminated surfaces and put their hands in their mouths. Research is ongoing — this is an emerging field.
Are there laws against vaping indoors?
Yes. As of October 2026, 28 states and the District of Columbia include e-cigarettes in their smoke-free indoor air laws, prohibiting vaping in workplaces, restaurants, bars, and other public spaces. The FDA's federal deeming rule also prohibits vaping on federal property. Check our state flavor ban guide for state-by-state restrictions. Many private businesses, landlords, and hotels also prohibit indoor vaping through private policies.
FDA Regulatory Notice: E-cigarette products are regulated by the FDA Center for Tobacco Products. The FDA recommends keeping e-cigarette products away from children and pets. Check the FDA-authorized product list for compliant products.
